You'll have to be a Guardian first. Usually they'll open some sort of form like this one on the forums where you need to tell them some information about yourself. There are also several requirements, for example you have to be at least 16 years old.

If you see any broadcasts about moderator forms, THEY ARE ALL FAKE PHISHING SITES.
