The Pet Trainer Medal is an item added in the Pet Battles update. It can be obtained by winning a Pet Trainer Battle. It can be used to purchase new various items from a Pet Trainer.
Having 200 Pet Trainer Medals in your backpack and successfully defeating a Pet Trainer after that will cause it to say "<Trainer Name> wanted to give you her Pet Trainer Medal, but you don't have room for it, so she left with it!"
