The Sack O' Joy is a back item which, when equipped, gives the player the Cheerful Giver mod. This mod forces the player to drop gems around as they move from their own balance.
If the player runs out of gems, no gems will drop out of the item. The player will not lose XP when dropping gems.
NOTE: Dropping gems into a Display Box or Transmatter Field no longer works due to a patch to prevent players from dropping and selling gems to other players.

The Sack O' Joy is part of the Santa Set (along with Pants, Hat, White Beard and Vest).
