The Death Scarf is a black scarf (which is a neck item) with 1 eye in front of it. It was released on the 2014 Halloween Week To get this item you have to sacrifice a Medical Scarf into the maw of Growganoth. It also spawns a Grim Reaper, which has a double sided scythe to follow you.
