When consumed, the player will receive the Brainworms malady mod for 12 hours, which can be cured through a successful surgery. Should the malady get cured, both players—the surgeon and patient—will receive a prize.

- On August 4, 2022, the amount of gems received from recycling a Vile Vial - Brainworms was changed from 250 to 0.
