During the Carnival, you can obtain this item from playing and winning the Brutal Bounce, which replaced the Bouncy House.
There are different levels of items you can achieve from winning, the lowest (common) is the Carnival Spikeball, medium (uncommon) is the Carny Vest, while the most rarest (rare) item to obtain is the Flame Scythe.
