It seems to be a stick with a huge ice block on the end. Whenever you swing this, the ice block flies off at incredible speeds. Bet it could do some serious damage.

When equipped, the Hammer Of Winter grants the Punch Damage: Hammer mod, which allows the player to break blocks faster. It also replaces the player's punch effect, causing them to throw the hammer instead.
