The Train-E is an unsplicable non-solid foreground block which was added as part of the Blocktor's Orders update on June 13, 2019.

Function

The Train-E can help guide players on performing surgery; however, it will only reward the player with Surgical Tools and increase the player's Surgeon Skill to 20 only. Completing surgeries on a Train-E whilst being over Surgeon Skill 20 will not increase it.

Trivia

• As of Player Appreciation Week 2020, Train-Es no longer give Caducei or give points during the Surgery Stars Clash Event.
• Train-E and Surg-E are references to the 2008 film Wall-E.
