The item may emit one of these types of signal rings which tell how far the player is from the source of radioactive point in the world:

Upon reaching the hidden source of radioactivity players will receive either one the following items :

When a Dead Geiger Counter is held, players will have an effect called: "Charging Geiger Counter"
*Uranium Block (doesn't use up the Geiger Counter's power, but doesn't count for quests either)

After finding the source, players will have an effect called "Irradiated." This means that they won't be able to find any radioactivity for 30 minutes. At the same time, your Geiger Counter will be replaced with a Dead Geiger Counter. During Geiger Day, the "Irradiated" mod only lasts for 15 minutes.
Trivia
*When the item was originally released, players could only get Radioactive Chemicals and Nuclear Fuels from using a Geiger Counter. Since the Crystals update, crystals became possible rewards, along with other items.
*As of Cinco De Mayo Week 2017, a Geiger Counter can only be used in regular worlds of size 100 x 60 (54 excluding bedrock). Therefore, it cannot be used in worlds that have been modified in size.
*During a Night Of The Comet (or in a world with an active Weather Machine - Comet), all Geiger Counters will emit green rings only since the comet messes up with their reading due to cosmic radiation. *Only one source are generated at a time. Two or more player holding Geiger counter at the same time will receive signal from the same source.

*Using the Geiger Counter in GEIGERHILLS will limit the rewards and you will only be able to get Uranium Block, additionally you won't get the "Irritated" mod.

Note: If the Geiger Counter is during the Night of The Comet Event, it will always emit green rings but not the rapid version of it, because the comet's radiation is messing up the counter. If players manage to find one, they will get the achievement: Atomic Luck.

Function

   You are detecting radiation...
   Geiger Counter removed.

When equipped, the Geiger Counter grants the Geiger Counting mod, which allows the player to locate radioactive items.

The item may emit one of these types of signal rings which tell how far the player is from the source of radioactive point in the world:

 • No rings - Very far.
 • Red rings - 15-30 blocks away from the source.
 • Yellow rings - 7,5-15 blocks away from the source.
 • Green rings - 4-7,5 blocks away from the source.
 • Rapid green rings - 0-4 blocks away from the source.

Once the player stands for a few moments on the source of radioactivity, the item they found will be added into their inventory. If the player's inventory is full, the item will be dropped on the tile they are standing. The player will then receive the Irradiated mod for 30 minutes (15 minutes on Geiger Day) and their Geiger Counter will turn into a Dead Geiger Counter.

The list of all possible rewards can be seen below.
